The Hospital Social Work Team cover the whole of the Middlesbrough area. The service is very keen to promote flexible working for all staff. This includes agile working at home as well as Middlesbrough Council and Trust bases.

The role will involve working to support Discharge to Assess Pathways and promote safe and timely discharges from hospital and following the client up in the community to ensure they are safe, have an effective support plan, and look to maximise their potential.
