Are you looking for a new challenge, working with short term cases in a supportive team? If so we are currently seeking to recruit a Social Worker to work within the discharge pathway at The James Cook University Hospital.
The Hospital Social Work Team provides a social work service to Adults and Older People in the Middlesbrough and Redcar and Cleveland areas, working with service users and their carers, providing advice and information and arranging services to support a safe discharge from hospital.
We are seeking applications for a social worker to work with wards 25, 34 and 36, focussing on patients who are on the fractured neck of femur pathway and those who are admitted to the trauma wards. The role will involve:
Providing a social work service to residents of Middlesbrough and Redcar and Cleveland
Liaising with allocated social workers and other local authorities to facilitate discharge planning
Working as part of the ward team and participating in the daily handovers/board rounds
Promoting the social work role and sharing knowledge of service provision and resources with other members of the ward team

We will support you by providing regular supervision, a manageable caseload and access to training.
