The Pathways Team have an exciting opportunity to recruit a full time Social Worker.
Social workers within the Pathways Team are responsible for working with young people aged 15 to 25 years old, who are looked after and will support their transition into adult hood. You will be a crucial person in the young people’s lives and support them to be the best that they can be. Building relationships with our young people is critical in this role to support young people to develop to adulthood.You will provide an assessment of needs for young people who are preparing to leave care and the development of a pathway plan to meet these needs.
